If your loved one is experiencing problems living independently, we’re here to help. Catholic Charities Adult Case Management Services works with seniors and their families to assist aging adults in maintaining safe, quality, independent living for as long as possible. We base our hourly rate on a sliding fee schedule for our Private Case Management Services.

Children and spouses of frail older adults often find themselves unable to cope with the many needs of their aged loved ones. A nursing home is not the answer they seek. Rather, help with living tasks, safety, medical and mental health needs, transportation, and contact with others can often make the difference between moving loved ones into nursing homes and maintaining them in their own homes.

That’s where Catholic Charities comes in. Our professional and experienced senior case managers can provide the assistance that makes the difference. We work one-on-one with seniors and family members to address concerns and difficulties, then provide the resources to help with the problems. If outside sources are needed, we co-ordinate the provision of those resources and make sure they are done.
