What We Offer

  • Be the extra pair of eyes to make sure the senior's needs are addressed
  • Conduct a home safety assessment and arrange for corrective changes.
  • Arrange doctors' appointments, assist in follow-through with physicians' care plans, and  provide necessary medication monitoring systems.
  • Monitor health and functioning abilities.
  • Coordinate and monitor the provision of home health care, homemaker and companionship services, and home safety and maintenance.
  • Arrange transportation to doctors' and other appointments.
  • Help with grocery shopping, housekeeping, cooking and laundry.
  • Fill out forms and assist with Medicare and Medicaid eligibility.
  • Provide home visits and companionship.
  • Keep in close contact with family for updates on the loved one's safety, progress, problems and functional abilities.
  • Provide consultation around financial services.
  • Facilitate the transition into a higher level of care, if warranted.
  • Monitor residential care plans and facilities and senior's care.
  • Provide a bilingual staff member (English/Spanish) for seniors who are mono-lingual
